Case Manager / Job Coach 80 – 100 % (m/w/d)

Aufgaben:

  • Individuelle, zielgruppengerechte Begleitung von versicherten Personen des RAV, der IV sowie den Sozialdiensten, in ihrem Integrations- und Förderprozess mit dem Ziel der Wiedereingliederung im ersten Arbeitsmarkt
  • Durchführung von regelmässigen Coachings, Standortbestimmungen, Interventionen,
  • Beurteilungsgesprächen, wie auch Entwicklung von Eingliederungsstrategien
  • Mitarbeit im Abklärungsmonat/Potentialabklärung (Fallführung, Analyse Stellensuche)
  • Aktive Akquisition von teilnehmergerechten Einzelarbeitsplätzen im zweiten Arbeitsmarkt, wie im ersten Arbeitsmarkt
  • Dokumentation des Prozesses inkl. Schreiben von Berichten und Arbeitszeugnissen
  • Zusammenarbeit mit Gruppenleitenden, Bewerbungswerkstatt und internen Stabstellen
  • Zielgerichtete Kommunikation und Kooperation mit externen Stellen wie AMM/RAV, IV, Sozialbehörden, Einsatzplätzen, Arbeitgebern und Fachstellen
  • Mitarbeit im Team und in Fachgruppen

Unsere Anforderungen an das zukünftige Teammitglied:

  • fundierte Aus- und/oder Weiterbildung im Sozialwesen, in Supported Employment, Case Management, Coaching oder Erwachsenenbildung
  • Erfahrung in der beruflichen Integration
  • Freude an der Akquisition von Arbeitsplätzen
  • sehr gute mündliche und schriftliche Ausdrucksfähigkeit
  • sehr gute MS Office-Kenntnisse
  • durchsetzungsstark, teamfähig, dienstleistungsorientiert und kreativ
