**Case manager**:

- Principale organisation faîtière du domaine du handicap en Suisse, Pro Infirmis et ses quelque 1900 collaboratrices et collaborateurs œuvrent au quotidien au respect de la dignité et de l’inclusion des personnes en situation de handicap. L’association s’engage particulièrement pour leur indépendance, leur autonomie et leur participation active à la société.
- Le dispositif Mon Plan a pour objectif de permettre la mise en place d’un projet de vie à domicile de personnes présentant une déficience intellectuelle et/ou un trouble du spectre autistique.

**Case manager (à 60 %)**:
**Vos missions**:

- Elaborer le projet de vie à domicile avec le-la client-e
- Mettre en place et assurer la coordination du réseau de soutien répondant aux besoins du-de la client-e
- Assurer le suivi et l’évaluation du projet de vie à domicile
- Contribuer aux activités de développement de la prestation
- Collaborer avec les partenaires externes et internes.

**Votre profil**:

- Bachelor en travail social ou formation équivalente
- Formation en Case management souhaitée
- Expérience dans le domaine du handicap et bonne connaissance du réseau éducatif et médico-social vaudois
- Bonnes compétences relationnelles
- Capacité d’analyse de situations complexes et de travail en réseau
- Capacité organisationnelle et esprit d’équipe.
