Job overview
The post holder will be the key person identified for providing cancer information and support, to enable people to navigate the health and social care system and make choices that are best for their cancer and their life. The aim of the role is to make sure that people living with cancer can access the right support, at the right time.
Advert
- Participate in patient and public involvement activities to promote the service including awareness and promotional events outside of the centre.
- Present / attend cancer conferences locally and regionally as required.
- Organise the day to running of the service, organising meetings, bookings rooms, responding to email enquiries.
- Ensure compliance with policies, procedures and clinical guidelines for self and others.
- Work with other Co-ordinators across the region to adopt best practice.
- Use databases and computer packages to initiate and maintain records and generate statistics for analytical purposes.
- Coordinate the handover with other teams to facilitate safe and effective transition of care between services in order to provide seamless support for people.
- Document and monitor all aspects of care coordination and service delivery, supporting data collection for audit.
- The post holder will demonstrate excellent organisational skills.

If the post you are applying for requires a Disclosure and Barring Service (DBS) check, please note that, in line with Trust policy, the cost of submitting and processing the DBS application for successful applicants will be met by the individual.
For substantive employees, the DBS costs are recovered through salary deductions following commencement in post in 3 equal instalments.
For bank workers and individuals appointed to honorary positions that require a DBS check, the cost must be paid in advance via the Trusts payment link before a start date can be confirmed.
Please note that DBS fees and any associated administration charges are subject to periodic review and may change from time to time.
The cost of the DBS check would vary depending on the level required for the role:
- Standard Disclosure £21.50 (DBS Fee) +£6.50 (Administration Fee)
- Enhanced Disclosure £49.50 (DBS Fee) +£6.50 (Administration Fee)
UK nationals or individuals with the right of abode typically meet the residency requirement if they have been ordinarily resident in the UK, the British Overseas Territories, or the Crown Dependencies (Channel Islands and Isle of Man) for at least three years prior to the start date of the apprenticeship.
