Dans le cadre de votre fonction vous serez amené(e) à :- apporter une aide sociale intégrée au processus de prise en charge biopsychosocial du CNP, en pluridisciplinarité et en collaboration avec l'extérieur
- orienter le patient dans le contexte de la santé publique en préservant ses intérêts et en favorisant son autonomie
- mettre en place la prise en charge en tenant compte des ressources personnelles des patients et de leur entourage
- mener des entretiens individuels, de familles et de réseau, en collaboration avec les soignants et / ou intervenants extérieurs
- participer à l'évaluation diagnostique globale et interprofessionnelle d'une situation de crise
- accompagner le patient dans des démarches et visites à l'extérieur de l'institution
- participer aux différents colloques institutionnels ainsi qu'à des groupes de travail
- entretenir et développer vos connaissances de bases en suivant des formations internes et externes
- être titulaire d'un Bachelor of Arts HES-SO en travail social ou titre jugé équivalent et reconnu par le Secrétariat d'État à la formation, à la recherche et à l'innovation (SEFRI)
- expérience professionnelle de 2 ans minimum en travail social
- permis de conduire et véhicule privé indispensables
- capacité à travailler de manière autonome et en équipe, dans un contexte de pluridisciplinarité
- faire preuve de créativité, responsabilité et flexibilité
- bénéficier d'excellentes capacités rédactionnelles et d'organisation
- montrer de l'aisance dans l'utilisation des outils informatiques usuels
- avoir une connaissance du réseau socio-sanitaire du canton
Entrée en fonction : 1er avril 2025 ou à convenir
Lieu de travail : mobilité possible sur les différents sites du CNP (Préfargier, CPCL et CPCM)
Type de contrat : contrat de durée indéterminée
