Die Fachstelle Alter berät unter anderem ältere Menschen und ihre Angehörigen rund um das Thema Alter. Die Stadt Uster möchte zukünftig ältere zuhause lebende Menschen und ihre Angehörigen in komplexen Situationen noch besser begleiten und unterstützen.

Die Fachstelle Alter der Abteilung Gesundheit sucht per 1. Januar 2024 oder nach Vereinbarung eine/n

**Case Manager/-in im Altersbereich 50%**:
**Ihre Hauptaufgaben**:

- Aufbau eines Case Managements
- Eigenständige Durchführung von systematischen Hausbesuchen bei Seniorinnen und Senioren sowie Vornehmen einer Situationseinschätzung vor Ort
- Begleiten von Seniorinnen und Senioren und betreuenden Angehörigen in komplexen Situationen
- In komplexen Situationen: Übernahme der Fallführung und Durchführung von Rundtischgesprächen (Case Management)
- Unterstützung der Leitung Fachstelle Alter bei der Information der Bevölkerung im Bereich der Prävention, der ambulanten und stationären Pflegeversorgung und altersgerechtem Wohnen
- Unterstützung der Abteilung Gesundheit bei der Umsetzung und Weiterentwicklung der Altersstrategie

**Das bringen Sie mit**:

- Ausbildung als Pflegefachperson
- Ausbildung oder fundierte Erfahrung in sozialer Arbeit, ausgewiesene Beratungskompetenz
- Weiterbildung in Case Management und Gerontologie
- Erfahrungen in Angehörigensupport oder Demenz von Vorteil
- Kenntnisse der aktuellen Entwicklungen im Schweizer Gesundheitswesen
- Gute Organisationsfähigkeit und PC-Anwenderkenntnisse
- Fremdsprachen von Vorteil
