Comptant près de 1’200 collaborateurs, les hôpitaux et les Medicentres du Réseau de l’Arc couvrent les besoins médicaux d’une population de plus de 50’000 habitants et se caractérisent par leur dynamisme, leurs excellentes infrastructures et leur souci constant de développer des prestations de proximité et de qualité.

**Afin de renforcer notre service de Case management, nous vous proposons le poste de Case manager**

**Description du poste**:

- Optimisation des places au sein de l’institution par la gestion des lits, principalement pour le site de Saint-Imier, mais aussi ponctuellement pour le site de Moutier (remplacements à effectuer sur le site de Moutier)
- Optimisation du flux patient en collaboration avec les médecins, les équipes soignantes et équipes multidisciplinaires, en tenant compte des SwissDRG et des actions suivantes:

- Réalisation du pré-codage des patients hospitalisés,
- Visites médicales pour ajustement des pré-codages et suivis des projets de sorties
- Collaboration avec les médecins pour l’identification des projets de sortie (retour maison, institutionalisation, réadaptation, )
- Remplissage des demandes de réadapatation
- Aide à la documentation médicale de sortie (lettre de sortie)
- Aide au développement et amélioration de la fonction

**Qualifications**:

- Diplôme d’infirmier de 3 ans, ayant de l’expérience dans différents domaines du monde hospitalier,
- Excellente gestion du stress
- Connaissances cliniques affirmées
- Connaissance des outils informatiques courants
- Conscience professionnelle
- Compétences relationnelles, aptitude à travailler en équipe
- Compétences communicationnelles
- Autonomie et sens de l’organisation et de la coordination
- Aptitude à prendre des initiatives avec sens des priorités
- Flexibilités dans les horaires
- Sens de la diplomatie
